Backfill the governance capability catalog from an app's current exposures.

Inputs: actor, organization_uuid, identity_app_uuid (required). Registers vw:<composition_uuid>:invoke for every exposed composition. Idempotent. Outputs: reconciled (count of exposed compositions processed), composition_uuids.
